Patient Account & Insurance Information ![]()
There have been changes in health care over the last decade due in part to the introduction of managed care. We
have established specific and efficient billing policies to help eliminate problems that often result from the
myriad of changes that have occurred. The policies have been designed to help you handle the issues that need
to be understood and addressed. The billing policies listed below should help you with the insurance filing
process at Osler Medical. We will file both your primary and supplemental insurance. We file the supplemental portion as a courtesy to you and if this is not paid within 60 days, it will be your responsibility to pay the balance due on the account and follow-up with the insurance company. We will handle the follow-up with the insurance company on the primary claims only. Please be sure to bring your insurance card to your appointment and provide it to the secretary to make a copy for our files. If the card does not have the claims address on it, please bring that information with you as well. Please be aware that you will be responsible for any unpaid balances due to erroneous or incomplete information. PLEASE BE PREPARED TO PAY ANY CO-PAYMENT, DEDUCTIBLE OR COST SHARE AT THE TIME OF THE VISIT. If you have a managed care plan (PPO, HMO) that requires an authorization or referral to see a specialist, please verify that the appropriate approval has been established prior to your visit. Also, be sure that the physician you are scheduled to see is a participant in your managed care plan. If you do not have the appropriate authorization from your insurance company, you will be financially responsible for charges related to the visit. It is important for you to realize that insurance companies will not issue authorizations after a visit. If at any time your address, insurance information, employer or any other information changes, please notify the office immediately. This will prevent us from submitting claims to the wrong insurance company, sending correspondence to an incorrect address, etc. If you do not have insurance, we expect payment at the time of service for any visits, lab or x-rays. We will be happy to set up payment arrangements for the billing of any extensive diagnostic testing. It is located on the 4th floor, Suite 402 of the Osler Medical building on US 1 in Melbourne.
